Saint Francis Ministries provides Permanency Case Management services in two states where we serve, Kansas and Texas.

Kansas Case Management

In Kansas, SFM provides comprehensive case management and permanency services to children and families through our Reintegration/Foster Care/Adoption (RFCA) programs. Local Department for Children and Families offices refer children in need of out-of-home placement. Each child and family’s situation and needs are immediately assessed, and children are placed in the least restrictive environment possible while maintaining connections with family, kin, school, and home community. Efforts are made to engage both identified parents. Most children are reunited with their birth families; however, those who cannot be reintegrated move on to adoption, permanent custodianship, or independent living.

Texas Case Management

In Texas, SFM provides stability, permanency, and safety to children and families as a Single-Source Continuum Contractor. We work with more than 130 agencies across Texas to ensure children are placed in safe and caring placements and families are able to receive services to address the concerns which led to their children being removed from the home. Our work in Texas includes creating long-term capacity to keep children in their counties of origin, creating permanency through adoption or reunification, providing independent living services to youth and young adults who will age out or have aged out of foster care, working with kinship and ICPC homes to help them meet licensing requirements, and ensuring the safety of children in out-of-home care.

1. Assessment and Placement:

  • Assessing and finding an appropriate placement that meets the child’s need. Kinship is always preferred and utilized, when possible.

2. Comprehensive Case Plan/Plan of Service:

  • Developing a comprehensive case plan/plan of service in collaboration with the child, biological family, foster or kinship parents, and other relevant parties.

3. Monitoring and Support:

  • Conducting regular visits and check-ins with the child in the foster home to assess progress, monitor safety and stability, and address any issues.
  • Providing support services such as counseling, therapy, educational support, and medical care as needed.

4. Advocacy and Coordination:

  • Advocating for the child’s best interests within the foster care system through collaboration with professionals, agencies, and stakeholders involved in the child’s life, such as teachers, social workers, and healthcare providers.

5. Transition and Permanency Planning:

  • Planning for the child’s transition either back to their biological family, adoption, or another permanent placement.

6. Documentation and Reporting:

  • Keeping detailed records of assessments, case plans, progress notes, and other relevant documentation.

7. Crisis Management:

  • Responding to crises or emergencies affecting the child or their placement and implementing safety plans and interventions when necessary to protect the child’s well-being.
